If we are not contracted with your insurance provider, payment is due at the time the services are rendered, and you
will be responsible for filing with your insurance company.

On your first visit, we ask that you bring with you the following: - Health insurance cards
- Photo
ID
- Your primary care doctor's contact information
- A list of any medications you are currently taking (prescription
and over-the-counter)
- A referral from your primary care doctor (if required by your insurance carrier)
- Any
previous x-rays, MRI's or test results relating to any past foot health conditions
